Win Physical Therapy is located in Hoffman Estates, IL. We are currently sharing space with Yama Physical Therapy. When you arrive at 1721 Moon Lake Blvd, please enter the building and take the elevator to the 2nd floor, suite 220.
Please fill out the online patient intake which was sent to your email prior to arriving for your appointment. This includes taking a picture of your insurance card (if applicable), doctor's prescription (if applicable) and photo ID.
Arrive 15 min early for your first appointment
Wear loose fitting clothing and gym shoes. You will be moving and doing exercises on your first day!

Medicare (non-participating provider)
Medicare non-participating providers accept Medicare insurance, however the patient is responsible for paying the full cost of treatment at the time of service. The billing statement will then be sent to Medicare. The patient may be reimbursed by Medicare at a later date. Please note that the full cost of each session may not be reimbursed.
Yes! You would pay the self pay/cash based rate for PT services.
Some insurances will allow you to submit for reimbursement as an out of network provider. Win PT will provide you with a “superbill” to submit to your insurance.
Illinois has “direct access” for physical therapy, which means that you do not need a referral from a doctor to begin treatment. However, some insurances have additional criteria.
Self pay patients will not require a prescription.
Most BCBS plans do not require a referral, however there are some exceptions (typically labor group policies).
Medicare beneficiaries do not need a referral but are required to provide the name of their physician to sign their physical therapy plan of care.
Win PT provides 1:1 care for all patients. This means that the full hour is spent with a Doctor of Physical Therapy. Your session will include a variety of interventions which may include manual therapy (hands on treatment), fully supervised performance of exercises, feedback on exercise technique, progression of complexity/resistance, discussion of patient goals and progression of the home exercise program.
Many insurance based physical therapy clinics employ rehab aides and PT assistants to oversee exercises with the physical therapist treating multiple patients at once. At Win PT, your physical therapist is fully focused on you.
